BCBS Prefix lookup is a 3-character alpha prefix or alpha numeric prefix in the first 3 positions of the BCBS member’s ID number. The purpose of the BCBS Prefix lookup list is to assist the provider to determine the plan submission based on the prefix and location of the services.

BCBS Prefix Lookup also known BCBS Alpha Prefix finder, it is used to find or search the BCBS Plan and the member affiliated BCBS companies by using the 3-character BCBS prefix. In this way it helps the provider to find the correct affiliated BCBS companies to submit and reimburse the claims.
BCBS Prefix lookup is critical for inquiries regarding the member, including eligibility and benefits, and is necessary for proper claim filing.
- Whenever we are billing the claim to insurance companies, billing companies should enter the member ID along with the 3-character BCBS Prefix. It is very important for Billing companies to bill the claim with BCBS prefix accurately as it appears on the member ID card.
- Billing companies should always include the ID number including the BCBS Prefix on any credentials relating to services to ensure precise handling by the BCBS Plan.
A Member ID includes the BCBS prefix in the first three positions and all subsequent characters between 6 and 14 numbers or letters up to 17 characters total.

Tips we should follow while submitting the claim with BCBS prefix:
- Always use the BCBS Prefix on the members current ID card.
- If there is no BCBS prefix, do not create one or use the BCBS prefix from another member’s ID card. Doing this may cause delays in processing and handling the claims.
- If the card presented has no BCBS prefix, then follow the instructions on the back of the ID card for inquiries and claim handling.
Some of the BCBS member’s ID card do not have an BCBS prefix or alphanumeric for the following products and programs:
- Stand-alone dental products
- BCBS FEP
- Pharmacy and stand-alone vision when delivered thru an intermediary model.
Federal Employee Program Member ID card:
FEP doesn’t have 3-character alpha BCBS Prefix or Alphanumeric BCBS Prefix. FEP member ID begin with the letter R followed by numbers.
